2004 Armed Forces Day

After-Action Report

This form is provided to obtain feedback from military organizations that participated in Armed Forces Day/Week (AFD/W) activities as participants or event hosts.

All military organizations that participated in AFD/W must submit a completed After Action Report by June 17, 2005. Submission via the electronic form below is preferred. Submissions by e-mail, fax (703-697-2577) or U.S. mail (Armed Forces Day Coordinator, OASD-PA/DPCR, Room 1E776, 1400 Defense Pentagon, Washington DC 20301-1400) are acceptable. Commands may consolidate their reports.

The data collected from the AARs will be used to evaluate current AFD/W activities and policies and to determine future changes to enhance commemoration of AFD/W.
